Beginning September 17, 2021, the NCH Mobile Unit will be on our campus twice a month to assist in wellness checks, vaccines, asthma treatment and more!
NCH and Hamilton Local Schools are committed to the health and well-being of all of our students. As part of this effort, NCH will be sending the Mobile Care Center to our district throughout the 2021-2022 school year to provide health care services to your child. One Friday a month they will provide service our Elementary, Intermediate, and Middle School students, and another Friday during that month they will provide service to our High School students.
To receive any of their services, please complete this consent form and return them by uploading them to the Google Form listed below, the front desk of your students building, or by email to our District Nurse at [email protected] or the Director of Public Relations at [email protected] or the.
